ISPs under developmentA list of the ISPs which are currently under development, together with a brief background, can be found below. For ISPs which have been assured please see the Assured ISPs page. Child Development, Disability and Additional Learning NeedsThis ISP covers the exchange of information between health, social care, education, Police, voluntary and independent agencies, with the aim of more transparent and effective information sharing in provision of services for children with developmental, disability and additional learning needs. The ISP will cover the areas of Swansea, Neath Port Talbot and Bridgend. Children with Additional Needs: Community Development Team - Aged 0 - 4This ISP covers the exchange of information between health, social care, education, statutory, voluntary and independent agencies in the pan-Gwent area. The Community Development Team aims to support of the effective development, care and wellbeing of children aged 0 - 4 with additional needs and to support their families and carers. Domestic Violence (MARAC)The ISP covers the exchange of information between representatives of local police, probation, health, child protection, housing practitioners, Independent Domestic Violence Advisors and other specialists from statutory and voluntary sectors for the purpose of dealing with cases of domestic abuse that have been assessed as high risk. The aim of the Multi-Agency Risk Assessment Conferences (MARAC) is to combine up to date information about a victim and to discuss the options for increasing the safety, health and wellbeing of the victim, and where appropriate, their families. Developments are underway in the areas of Newport and Caerphilly, with the ISP for the Wrexham conference having recently passed the quality assurance process. Enhanced CareThe ISP cover the exchange of information between health, social care and the voluntary sector within Denbighshire (North); facilitating greater integration between the service providers with the aim of improving access and continuity of care for Service Users by enabling services in their own home. Flying Start ProgrammeThe Flying Start programme covers the exchange of information between health, social care, statutory, voluntary and independent agencies, with the aim of providing children in disadvantaged areas with a better start in life both in preparation for school and longer term. It aims to make a decisive difference to the life chances of children aged 0 – 4 years in the areas in which it is operational, by ensuring more effective joint working. Developments are underway in Ceredigion, Cardiff, Rhondda Cynon Taff, Merthyr Tydfil, Carmarthenshire, Wrexham and Conwy. Galw Gofal / Care Connect Call Monitoring ServiceThis ISP covers the exchange of information between the six Local Authorities, Local Health Board, housing providers and voluntary services across North Wales. Galw Gofal aims to provide an effective and accessible 24 / 7 emergency call monitoring service for Telecare and Out of Hours Services that promotes safety, security and independence for its Service Users across the North Wales region. Integrated Community Mental Health Teams (CMHTs)The CMHTs aim to provide quality mental health assessments and interventions with service users and carers, in a manner that promotes good mental health. Improving communication and reducing duplication of work will enable the provision of safe and efficient systems for assessment, care and treatment. The ISP will support both health and social care partner organisations in sharing personal information through these multi-agency teams, across Cardiff and the Vale of Glamorgan. The ISP is currently going through the Quality Assurance process. Integrated Family Support Service (IFSS)The ISPs cover the exchange of information between health, social care, housing, education, youth offending, probation and the voluntary sector for the purpose of delivering services, for and in respect of children and young people under 18 years of age who have been allocated a social worker. The service aims to enable parents and carers achieve the necessary behavioural changes that will improve their parenting capacity, whilst addressing the social, cultural and organisational factors which have an impact on the safe care of the child or young person and their parents. The Integrated Family Support Teams work to keep families together in a safe environment wherever possible by promoting safety, better outcomes and seamless service provision. ISPs are currently being developed in Pembrokeshire, Camarthenshire, Ceredigion and Cardiff. ISPs already developed for this service are available on the Assured ISPs page. Integrated Young Peoples Substance Misuse ServiceThe Integrated Young Peoples Substance Misuse Services has developed an integrated approach to supporting young people and their families where there are indicators of emergent substance misuse issues. It aims to integrate and enhance existing services to meet identified unmet priority needs for those young people. The ISP covers the exchange of information between the voluntary sector, social care, youth offending, health and education; for the purposes of providing multi-agency services for young people, between 11 and 18 years of age across Ceredigion, where substance misuse is an issue. Keeping in Touch (KIT)This ISP covers the exchange of information between Careers Wales and further education institutions in Wales for the purpose of identifying those young people who are at risk of leaving full time education courses early, in order to provide them with support, advice and services intended to help prevent them from becoming either; not in education, employment or training (NEET). Management of children and young people with an advanced and emergency care planThis ISP covers the exchange of information between health, social care, police and children's hospices across Wales, with the aim of ensuring that children and young people who die at home with an Advance and Emergency Care Pathway (AECP) in place, are dealt with appropriately by all agencies to prevent additional distress to their family. When a child or young person is at risk of life threatening deterioration, an AECP is developed to agree on the most appropriate care in an emergency. The ISP is currently going through the Quality Assurance process. Mental Health Assertive Outreach ServiceThis ISP, currently under review, covers the exchange of information between Aneurin Bevan Local Health Board, Caerphilly County Borough Council, Wales Probation Trust and Heddlu Gwent Police in the Caerphilly area, with the aim to facilitate a co-ordinated approach to the planning and delivery of services. The aim of the Assertive Outreach Service is to engage those service users who have demonstrated a reluctance and/or inability to use mainstream mental health support services and who, because of their non-engagement with existing support services, are deemed at risk a deterioration in their mental health and therefore of being unable to live successfully in the community. Its primary function is to help Service Users to achieve the best possible quality of life whilst acknowledging the difficulties imposed by their mental health problems. This is done by maintaining the Service User in the community through active engagement, practical support and treatment including symptom management, solution focused interventions and social inclusion. Not in Education, Employment or Training (NEET)This ISP covers the exchange of information between education, social services, youth offending, housing, Careers Wales, Job Centre Plus, training providers and the voluntary sector in the Blaenau Gwent area. The NEET Multi Agency Service aims to provides a targeted response to support Service Users, between 11 and 25 years of age, to overcome personal barriers and back into education, employment or training. Protection of Vulnerable Adults (POVA) - Multi-agency strategy discussions and meetingsThis ISP covers the exchange of information between social care, police, health and inspectorate bodies at the strategy discussion and meetings stage of the adult protection process within Powys. The aim of the strategy discussion and meetings is to share relevant information each partner agency has about the vulnerable adult and alleged perpetrator, consider issues relating to the referral, in particular the level of risk, and decide on a course of action. The ISP is currently going through the Quality Assurance process. Public Protection Mental HealthThe Public Protection ISP covers the exchange of information between Heddlu Gwent Police, Aneurin Bevan Local Health Board, Caerphilly County Borough Council and Gwent Probation Services with the aim to facilitate the Caerphilly (North) Mental Health Adult Services, in reducing the risks to the community by safeguarding people and promoting their welfare. It aims to alleviate crime, anti-social behaviour and to ensure the proper management of offenders who are deemed to provide a risk to the community, by providing assistance to the Service Users in enabling them to not commit offences or re-offend. Specialist Children's ServiceThis ISP covers the exchange of information between health, social care, education, housing and the voluntary sector when providing care and support in the form of a multi-disciplinary service to children and young people who are disabled or ill and their families in the Gwynedd locality. The ISP is currently going through the Quality Assurance process. Substance Misuse ServiceThis ISP covers the exchange of information between health, social care and drug treatment agencies with the aim of supporting the delivery of substance misuse services to service users and their carers living within the areas of Bridgend, Neath Port Talbot and Swansea. The service aims to facilitate greater integration between the service providers with the aim of delivering a consistent, co-ordinated and seamless service through timely assessment and safe and effective treatment. Supported Housing GatewayThis ISP covers the regular sharing of information between, local authority housing, private housing and third sector organisations with the aim of supporting a range of vulnerable people in Newport, in accessing the most appropriate housing to meet their needs. Team Around the Family (TAF) - Families First ProgrammeA model ISP has been developed to form the basis of other TAF ISPs across Wales. It details how personal information is shared across the model between Families First projects, as well as other key agencies which form part of a Multi Agency Panel of Lead Professionals. Details of this quality assured model ISP is available on the Assured ISPs page A similar development is due to commence in Cardiff shortly, using this model ISP. It's understood that other developments for TAF ISPs across Wales, will be underway in the near future. Transition to AdulthoodThis ISP covers the exchange of information between schools, the local authority, health and Careers Wales in the Merthyr Tydfil area. The information sharing aims to support young people aged 14-25, who have a Special Educational Needs Statement, Disability or are diagnosed with Autistic Spectrum Disorder, during their transition to adulthood.
